Wellbeing Practitioner – Patient Intake & Onboarding

Contract: Remote UK based, Permanent, Part-time
Hours: 20 hours per week (Service core hours are 9-5 Mon to Fri)
Salary: £21714 per annum (£38,000 FTE)
Location: Remote (Work from home) you must have the right to work in the UK.
Reports to: Wellbeing Team Manager

The Role
We're looking for experienced wellbeing practitioners to join our growing team and help shape the experience of patients starting their journey with Anne Health. You'll be responsible for conducting intake sessions with new patients, providing a warm, professional introduction to our services whilst gathering essential clinical and psychosocial information. You'll also provide ongoing counselling and therapy sessions for clients who would benefit from regular therapeutic support.

This role is central to how patients experience Anne Health. You'll create a safe, affirming space where people feel heard and understood, whilst ensuring we have the information needed to provide excellent, individualised care. You'll work collaboratively with our clinical and administrative teams to support smooth onboarding and ongoing patient wellbeing.


Key Responsibilities
  • Conduct intake assessments with new Anne Health patients, exploring their goals, needs, medical history, and psychosocial context
  • Provide ongoing counselling and therapy sessions for clients requiring regular therapeutic support
  • Deliver gender-affirmative therapeutic interventions tailored to individual client needs
  • Provide clear, accessible information about gender-affirming care pathways and Anne Health services
  • Complete thorough documentation of intake sessions and therapy notes in line with clinical governance standards
  • Identify and appropriately manage risk, safeguarding concerns, and complex needs
  • Collaborate with prescribers, care coordinators, and wider team members to ensure continuity of care
  • Support patients in understanding informed consent approaches to gender-affirming healthcare
  • Maintain professional boundaries whilst building rapport and trust with patients
  • Manage a caseload of therapy clients alongside intake responsibilities
  • Participate in regular clinical supervision, team meetings, and reflective practice
  • Contribute to service development and quality improvement initiatives
  • Keep accurate records in accordance with GDPR and professional standards


Essential Requirements


Professional Qualifications & Registration
  • Recognised qualification in counselling, psychotherapy, psychology, social work, or related mental health field (usually Diploma level or higher)
  • Registration or accreditation with a relevant professional body (e.g. BACP, UKCP, HCPC, BABCP or equivalent)

Specialist Knowledge
  • Demonstrated knowledge of gender diversity, transgender identities, and gender dysphoria
  • Understanding of gender-affirming care pathways, including hormone therapy, surgery, fertility considerations, and social transition
  • Familiarity with WPATH Standards of Care and current best practice guidelines

Clinical Skills & Experience
  • Clinical experience working with LGBTQ+ and gender-diverse clients
  • Competence in mental health assessment, risk assessment, and safeguarding practices
  • Experience delivering therapeutic interventions and managing a therapy caseload
  • Knowledge of ethical, legal, and equality frameworks relevant to transgender healthcare
  • Strong therapeutic skills including empathy, cultural competence, and affirmative practice


Professional Practice
Commitment to ongoing supervision, reflective practice, and continuing professional development
  • Excellent written and verbal communication skills
  • Ability to work independently whilst being a collaborative team member
  • Comfortable working remotely with strong IT skills
  • Effective time management and ability to balance intake work with ongoing therapy commitments


Desirable
  • Experience working in healthcare settings or with medical professionals
  • Knowledge of informed consent models of care
  • Experience with diverse patient populations including children, young people, and adults
  • Understanding of intersectionality and how multiple identities shape healthcare experiences
  • Training in specific therapeutic modalities (e.g. CBT, ACT, person-centred, psychodynamic)
