An exciting opportunity has arisen within the Regulated Care Team at Northamptonshire Carers. Our Regulated Care service provides emergency and planned short respite for unpaid carers as well as providing vital support to individuals overnight who are transitioning from hospital to home. We also manage a Community Companion service which offers volunteer befriending support, allowing carers to have a regular break from their caring role.  This is a dynamic, fast-paced role that requires excellent organisational, communication and problem-solving skills.

As a Care Coordinator, you will play a key role in managing referrals, coordinating care visits and volunteer visits; supporting with staff observations and spot checks in the community as well as client assessments, ensuring high-quality, person-centred support for individuals living in their own homes. Working closely with colleagues across Northamptonshire Carers, healthcare professionals, social care teams, unpaid carers and families, you will help deliver responsive and effective care services across the county.

Due to the collaborative nature of the role, office-based working is essential, along with travel throughout Northamptonshire. Participation in the out-of-hours on-call rota is required and appropriately remunerated.

Key Responsibilities

Care Coordination and Service Delivery

  • Manage referrals across Regulated Care service areas, including emergency and planned respite, overnight hospital support, private care visits and volunteer visits.
  • Coordinate care and volunteer provision from referral through to assessment, care planning, scheduling and ongoing review.
  • Organise and schedule care visits, ensuring all required documentation and assessments are completed.
  • Liaise effectively with hospital discharge teams, Adult Social Care, GPs and other professionals to facilitate seamless care visits.
  • Conduct home visits, welfare checks and staff observations to ensure safe and effective service delivery.
  • Monitor care planning documentation, identify gaps and ensure records are completed accurately and promptly.
  • Respond to the needs of vulnerable carers and clients through welfare calls and ongoing support.
  • Assist with care delivery where operational needs require.
Compliance

  • Ensure compliance with Care Quality Commission (CQC) regulations, GDPR and all organisational policies and procedures.
  • Support the effective operation of regulated care services across both day and night provisions.
  • Maintain high standards of care quality and readiness for inspections, audits and quality assurance reviews.
  • Produce and maintain accurate records, statistics and reports for management purposes.
  • Ensure all client and staff records are securely maintained in accordance with data protection requirements.
  • Promote safe lone-working practices and adherence to health and safety legislation.
  • Participate in on-call duties and provide operational support as required.
  • Build and maintain strong relationships with external agencies and multidisciplinary teams to enhance service provision across the health and social care sector.
  • Undertake additional duties as required by the Registered Manager, CEO, COO and Trustees.
Staff Management and Support

  • Oversee staff annual leave, sickness absence recording and related procedures.
·       Ensure staff are appropriately trained, competent and operating in line with quality standards.

  • Support the personal and professional development of allocated supervisees.
  • Promote equality, diversity and inclusive working practices throughout the service.
Training and Development

  • Attend relevant training courses, workshops and development opportunities.
  • Ensure staff training and development plans remain current and compliant with CQC requirements and industry standards.
