Job Summary
We are looking for a motivated and reliable individual to join our Ageing Well team and support the delivery of our Community Asset Groups and Memory Hubs. These groups enable people living with long-term health conditions such as Dementia, Diabetes, Heart Failure, Fibromyalgia, Acquired Brain Injury, and their Carers to stay well, stay connected, and feel supported.

This is a rewarding and responsible role, ideal for someone with experience in health and social care, group facilitation, or community-based work. We also welcome applicants who bring lived experience of long-term health conditions or caring responsibilities, as this valuable insight brings important perspective to the support that we provide. 

In this role, you will:
  • Set up, lead, and promote group sessions for people with long-term conditions, including Dementia, and their Carers.

  •  Support the delivery of services across North and West Northamptonshire. Whilst we will always seek to plan travel and workloads efficiently, flexibility to work across the county is essential to ensure carers and people we support continue to receive high-quality services, particularly during periods of staff absence and vacancies. 

  • Work collaboratively with partners in health, social care, and the voluntary and community sector.

  • Deliver engaging, informative, and meaningful activities that support people to live well and independently for longer.

  • Provide one-to-one support, either by phone or face-to-face, tailored to individual needs

  • Support and work alongside volunteers and peer supporters, many of whom bring valuable lived experience.

  • Represent our service at community and professional events, building awareness and community connections.

We are actively expanding our Community Asset Groups across Northamptonshire and play a key role in shaping and responding to both local and county-wide support. We are committed to reducing health and social inequalities by raising awareness of unmet needs and using our creativity and resources to find practical, community-led solutions.

This role is part of a multi-disciplinary, multi-agency approach, and involves working closely with professionals, volunteers, and community members. Success in this role depends on being approachable, reliable, and responsive—someone who listens actively, communicates clearly, and follows through on commitments. We value creativity, teamwork, and co-production—working together with people we support to design services that truly meet their needs.

An Enhanced DBS check is required for this role

Mileage paid at 45p per mile, Pension Scheme, Simply Health. 24 days annual leave (pro rata)

Notes

We are looking for 2 candidates to fill 2 roles, one for 22 hours, Wednesday to Friday and another 30 hours Tuesday to Friday.
