About the Service
Our Veterans Carer Support Service works with families of wounded, injured or sick serving personnel and Veterans. We aim to deliver early intervention and prevention as quickly as possible, breaking down barriers with key services and help ensure the needs of carers and the people they support are fully considered. We also act as an advocate for individual carers, encourage them to advocate for themselves and support with recognising the importance of self-care.
About the Role
As an Outreach Worker, you will be working in the Angus locality supporting veterans of carers who are affected by mental illness. Focusing on one-to-one support you will carry a case load to develop and review personal support plans to help achieve person centred and National Health and Wellbeing outcomes. You will support people in their homes and within the Angus community, you will also facilitate a peer support group for Carers of Veterans.
You will support the Service and Head of Service, in their strategic role to meet national and funding criteria and liaise with other statutory and third sector agencies to make appropriate signposting as needed and to raise awareness of Change Mental Health services. You will work closely with partners in the community to identify gaps in the service where they arise and implement solutions in partnership with your line manager.
You will work autonomously, being able to judge when to refer to your manager and take a lead role in developing the service. You will have a lead role in developing the service.
Key Responsibilities:
· Work closely with the Service Manager and Head of Service to achieve strategic outcomes
· Provide support to people one-to-one and in group settings
· Establish supportive face to face relationships with each individual, encouraging individual responsibility and decision-making to maximise choice, control and self-management
· Co-create and co-review service user development plans, ensuring outcomes are being achieved, including people are connected to their community
· Maintain accurate records and ensure confidentiality is maintained for people using the service
· Ensure that careful and ongoing reviews are maintained and manage individuals’ off-boarding appropriately in a timely manner
· Ensure that people you support are connected to other Change Mental Health services in the area as appropriate
· Research and develop tools in response to unmet need and assist in Mental Wellbeing
· Ensuring continuity of service within the context of Change Mental Health values and strategic outcomes
· Build positive working relationships with local statutory service, agencies and the local community to better support our service users and to promote the work of Change Mental Health
· Collate information and prepare reports for line management as required
· Ensure you are following all Health and Safety responsibilities
Essential Criteria:
· Educated to SVQ 3 level or have experience working in social care
· Understand the need for a compassionate, non-judgemental approach to supporting people
· Ability to work well in a team setting whilst also working autonomously
· A sound theory base of social care interventions including risk assessment and risk management
· A sound knowledge of Health and Social care policy in Scotland and ability to apply this to the role
· A value base consistent with the aims and objectives of Change Mental Health
· Take a person-centred approach and apply a solution focussed way of working to support the individual
· Competency in planning and reviewing development plans and the ability to take appropriate action if needed
· Competent in co-creating and reviewing development plans and taking appropriate action if needed
· Manage your own workload and diary planning
· Drivers licence and having access to a car as the expectations you will travel within Angus and one day a week visit the Perth and Kinross office for team meetings, training and development days
· I.T skills are of a good standard
· Keen to develop your knowledge and skills and attend training as required

· Ex Veteran, as this would bring a huge amount of lived experience, empathy and passion to the role
General Duties:
· Act in accordance with Data Protection legislation. Ensure all records, personal, staff and client data are managed in line with Data Management and Information Governance policies
· Comply with legal and regulatory requirements such as provisions set out in the Health and Safety at Work Act 1974
· As with all employees, workers and volunteers; to encourage people to join Support in Mind Scotland as a member, donor or activist
· To act in accordance with the charity’s Health & Safety and Safeguarding policies and to notify your line manager promptly if there are any concerns
· To participate in regular supervision and appraisal and undertake any relevant training as appropriate to the role
· To work in accordance with the charity’s national policies and local operating procedures and those of external regulators or professional bodies.
