We have an exciting opportunity to work for Bristol City Robins Foundation as a Health and Wellbeing coach (part-time), working in partnership with Bridge View Medial PCN.
 
The Health and Wellbeing Coach will work with a range of individuals identified by Bridge View Medical (such as those with Type 2 Diabetes, obesity, cardiovascular disease, stroke, hypertension, anxiety and persistent pain), guiding and supporting them to change their lifestyle behaviours to promote better health, wellness and overall quality of life. 

Your main responsibilities:
 
·        Identification of people who would benefit from health and wellbeing coaching interventions.
·        Responsibility for providing support (non-clinical) to a cohort of patients who will benefit from proactive health management. 
·        Take referrals from the wider practice team including GPs, Practice Nurses, Social Prescribers, Clinical Pharmacists, Dietitian and Physiotherapists who will have identified patients that meet the criteria for Health and Wellbeing coaches.
·        Teaching and supporting patients and carers to understand and manage their own conditions and maintain an independent lifestyle through health coaching techniques to encourage patient activation.
·        Supporting the development of personalised patient care and support plans, liaising with the GP practice team, patient and carer and specialist teams as appropriate.

You will have:
 
·       PCI-Accredited Tailored Health Coaching Course (Desirable – or willing to work towards)
·       Experience of working in a public-facing setting (ideally a culturally diverse community)
·       Experience working with people experiencing mental or physical health challenges.
·       Be confident in coaching and motivating a caseload of patients through multiple sessions, identifying their individual needs, preferences, and abilities to effectively implement a personalised wellness plan to achieve goals and aspirations.
·       To possess the knowledge and skills to motivate and educate patients to engage in dietary and lifestyle changes to benefit health, for example, education regarding low-carb diets for diabetes. 
·       Provide personalised support to individuals, their families, and carers to ensure they are active participants in their healthcare, empowering them to take more control in managing their health and wellbeing, live independently, and improve their health outcomes.
·       A good understanding of the wider determinants of health and wellbeing and the ability to further support patients where required (refer them for further advice, guidance and advocacy). 
·       Utilise motivational interviewing, goal-setting strategies, and behaviour change theories to help clients overcome barriers and establish sustainable habits.
·       IT literate.
·       Full UK driving license.

Notes

This position is part-time (22.5 hours per week).

The candidate must be willing to work occasional evenings and weekends. The successful candidate will be subject to an Enhanced DBS check.
