The Carers in Bedfordshire Health Team are looking for a dedicated nurse who can carry out physical health checks.
The primary purpose of the role of Physical Health Nurse is to deliver a patient-focused programme of work to improve the physical health of people with a serious mental illness (SMI) and their carers. Your focus will be providing physical health checks and interventions for people with serious mental illness who may also have physical co-morbidities and their carers. You will be a Registered Nurse with experience of working with people with SMI and/or long term conditions.
A variety of soft skills and experience is required for the following role as you will be working with carers and the person they care for.
The new service is nurse-led work that will meet the needs of the service. The right candidate will be able to work flexibly over the week to meet the needs of carers and the person they care for, in the community and in patient homes. You will have a base at the Carers in Bedfordshire (CiB) office and be part of the busy Health Team.
The right candidate will have the enthusiasm, drive and motivation to get this new service off the ground and have a commitment to improving the Physical Health of People with SMI and their carers. You will be comfortable at engaging with people who experience health inequalities or difficulty accessing services and provide innovative solutions to overcoming barriers within the community.
The role requires close collaboration and liaison with Community Mental Health Team colleagues, as well as external partners, primarily GPs / primary care colleagues, other health providers, health promotion and voluntary sector organisations.
