So, a bit more about the role – Carers Health Lead:
To manage, deliver and develop all areas and contracts of Carer Health strategy, leading a team of carer link workers, annual health check nurse, sessional staff and volunteers. To oversee and work within the carers hubs across Bedfordshire Hospitals NHS foundation Trust and to effectively form relationships with NHS, primary care and the community mental health teams across Bedfordshire. 

Service Lead Delivery: 
  • Ensure service provision delivers on contractual and funding commitments while delivering an effective service to carers that delivers outcomes and delivers KPI’s (inc. Hospitals contract, Carer Link Worker, NHS Nurse and Carers Contract)
  • Problem solve and overcome barriers to professionals referring to CiB, manage conflicting demands. 
  • Contribute to service and organisational development including contributing to tender responses, funding bids, attending Board meetings on request and contributing to other areas of development and improvement by agreement with the CEO
  • Prepare quarterly reports for each contract and other funders as required 
  • Collect data and monitor and evaluate activities to ensure outcomes are achieved, and documented. Interpret trends from data to establish routes for progression and unmet need  
  • Where carer need is identified produce business cases that would meet demand 

Hospital Hubs: 
  • To establish and maintain relationships with key personnel in the hospitals to facilitate the identification and support of carers, including but not limited to the Patient Experience Manager, Discharge Teams, Frailty team, Nurses and Ward Staff, Outpatient Clinic Staff, Communications team and Volunteer Coordination team 
  • To ensure that unpaid carers are supported whilst in the hospital setting and once the person they care for has been discharged home or onto another establishment 
  • To maintain good contacts with other agencies offering support to carers and the person they care, including members of the PALS (Patient Advice Liaison Service) team and external bodies who offer support to carers 
  • To identify carers, register them, offer immediate support and onward referral where appropriate 
  • To maintain statistical records as necessary for the preparation of contract reports and other data requests 
  • Form effective relationships within professional boundaries with carer and members of other multi discipline teams.
